Freedom of Information Consent Form

Dear Parent(s)/Guardian(s):

Over the last few years, De La Salle has been working on several initiatives to make applications, registrations and payments more easily accessible and efficient for our parent community to use, while also trying to be more environmentally conscious. To date, we have been successful in implementing our online payments and forms, and therefore we will now move forward with the next phase which includes obtaining electronic signatures on various permission and consent forms.

Our first "trial run" will be the Freedom of Information Consent Form, which you will need to complete and submit for each child no later than Friday, October 5. If this process works, we will slowly begin to roll out electronic forms for grade-wide and school-wide activities, such as retreats, Winter Activity Day, etc.

To complete the Freedom of Information Consent Form, please follow the instructions below:

(You will be directed to log in to your Parent Portal account.)
Sign in to Portal as prompted. To satisfy legal requirements, at least one parent/guardian must be signed in to the Portal.
(All parents were given a Del Portal username and password when their child first started at the College. If you need a refresh on your username and password, or need your password reset, please email helpdesk@delasalle.ca with the subject line "Portal Log In Reset -Online Form" and your updated information will be sent to you promptly.)
Read the form carefully and complete the necessary fields.
Upon completion of the form, you will be asked to provide the email address associated with your Portal profile (the email where you received this communication.) This is an important final step as it will further authenticate the electronic signature.
